2009-2010 Paperwork needed to participate on the Team is required of all new and returning players.

* Failure to submit completed forms WILL delay you from team participation of all activities. That includes keeping copies of insurance cards, driver's license, and auto insurance up to date with the Sport Club Office!

 

Player Equipment/Options form must be completed (and optional items paid for) by Wednesday, Sept. 9th at 6:00pm! All checks to be made payable to "AALF". The next team order is done in January.
